Because of his pending retirement, Dr. Spaine is no longer scheduling appointments for new or returning patients (patients who have not been seen in the past year).

RECORDS REQUESTS

To assure that your protected health information remains private, all requests for medical records must be made in writing, using the appropriate authorization form.

Medical records

Medical records generally include documents relating to: your diagnosis, testing and assessments, general chart notes, correspondence, treatment plans, symptoms, types of treatment provided, functional status, and other items related to your treatment. Medical records do NOT include your therapist's private notes (psychotherapy notes) from your sessions.

Medical records may be requested by completing and returning a Authorization for Disclosure for Release of Information form.

NOTE: If you request that your medical records be sent directly to you (rather than to another provider or other third party) you must also complete a Release of Liability form.

Psychotherapy Notes

Psychotherapy Notes are the private notes your therapist takes during your sessions. These notes document and analyze the contents of your conversations during your sessions. Because these notes contain highly personal and private details of your sessions, they are kept separate from your Medical Records and may not be released without a separate authorization.

Psychotherapy Notes may be requested by completing and returning an Authorization for Release of Psychotherapy Notes form.

NOTE: If you request that your Psychotherapy Notes be sent directly to you (rather than to another provider or other third party) you must also complete a Release of Liability form.

Copying and Postage Fees In accordance with Arkansas Code Setion 16-46-106, and the Psychotherapist-Patient Services Agreement, we may charge a fee of fifty cents (.50) for the first twenty-five (25) pages, and twenty-five cents (.25) for each additional page as well the actual cost of postage to copy and mail patient records.


THIRD PARTY RECORDS REQUESTS

To assure the privacy of our patient records, all third party requests for medical records must be made in writing, and must include a HIPAA compliant release signed by the patient (or other duly authorized representative).
